HOW TO ENROLL

ENROLLMENT PROCESS

If you decide to move forward, our Intake Specialist will set up an informational meeting/presentation. This meeting can be in your home, or at the Fresno PACE facility. The Intake Specialist will review eligibility requirements and inquire about your needs and expectations, and answer any questions your caregiver(s) may have.
The next step will be a tour of the Fresno PACE facility. The tour gives you the opportunity to see and experience the services that Fresno PACE provides, as well as meet other participants and staff. We can provide transportation to and from the facility, lunch will be provided.
After the tour the Intake Specialist will schedule a home safety clinical visit by one of our Nurses. At this visit they will ask questions about your medical care, home care needs and living situation.

Once that assessment is completed, the second visit will take place at Pace with our clinic nurse to review information about your health care needs. We’ll provide transportation and lunch for that visit.
If you’re eligible and if you wish to enroll, our Intake team will meet with you and your caregiver(s) to review enrollment documents about the Pace Program. You’ll have an opportunity to ask questions and discuss your membership with Fresno Pace. You’ll also receive your Member Enrollment Agreement Terms and Conditions, sign your Enrollment Agreement and confirm that your caregiver(s) are able to provide for your care in partnership with Fresno PACE.

ELIGIBILITY CRITERIA

  • Are 55 years old and above
  • Reside in Fresno County
  • Require assistance with activities of daily living
  • Need a level of care equal to nursing home services
  • Can live safely in your community with the help of PACE services

 

Fresno PACE accepts Medicare and/or Medi-Cal. Fresno PACE members have no out-of-pocket costs for healthcare services, prescriptions or any of our services.

HOW PACE WORKS

The Interdisciplinary Care team (IDT) is what makes Fresno PACE unique. The IDT is the principal care management mechanism in PACE which directly provides and coordinates all care for our Members.

 

The team is composed of a primary care physician, center manager, registered nurses, physical, occupational and recreational therapists, social worker, dietitian, home care coordinator, certified aides, and a transportation representative.

 

It’s this team that provides direction on your health care needs 24-hours a day, 7 days a week. When a member needs healthcare or a service that supports a healthy lifestyle this group will coordinate and implement those services so that the Member can maintain their health and independence.

When you enroll with Fresno PACE, we will be your sole service provider and you agree to receive medical services exclusively from our organization, except in the case of an emergency or for urgently needed services. You will have access to all the care you need through our staff or through arrangements that Fresno PACE makes with contract providers, but you will no longer be able to obtain services from other doctors or medical providers under the traditional fee-for-service Medicare and Medi-Cal system. Enrollment in Fresno PACE results in disenrollment from any other Medicare or Medi-Cal pre-payment plan or optional benefit.

 

Participants may be liable for the cost of unauthorized or out-of-PACE program medical services from an out-of-network provider or without prior authorization with the exception of emergency services.
