Fresno’s Program of all-inclusive care for the Elderly (PACE) is a unique health care program that provides medical, health and wellness services to seniors. Fresno PACE supports senior’s living with chronic illness or disabilities so they are able to continue to live at home independently.

We provide a caring network of medical and social services which promotes independence, dignity and pride and enables our Participant to live at home in the community with friends and family. Fresno PACE eliminates disparities in health care access and outcomes by providing superior quality health and human services through an integrated world-class delivery system for multi-ethnic and underserved communities in Fresno County.

PACE is funded by Medicare and Medicaid to deliver necessary care. Private pay seniors who are not covered by Medicare or Medicaid may also be accepted. Upon enrollment in the program, the participants agrees to receive all health care services through PACE’s health care network. Participants may be liable for the cost of medical services from an out of network provider or without prior authorization with the exception of emergency services.
